technically has 3/3, but would be an spoiler to say why. Is both 1/3 and 3/3 in some perspective (spoiler category), although have to point out that most of the screentime is from both women actresses. Giving too much detail would be an spoiler, but would like to comment that this movie mostly tackles the body dismorphia issue on a femme perspective.
We mostly will focus on the story of our protagonist here, but from the overall movie, this could be considered both 1/3 AND 3/3, disagreeing and agreeing on the review, also confirming the dubious mention as a mark there

We mostly focus on the main character Elisabeth Sparkle. Tackles mostly (and heavily) on her inner world and self + issues (being ambiguous to not enter the spoiler category). 2 - 3 could be pointed in question, but tackles it on its own way of form, so at the same time it would also pass 2 and 3 too.

Women are the main characters: Elizabeth Sparkle and her younger, prettier alter-ego created by the Substance, Sue. But they never have a conversation, only at one point tearing at each other as they fight to exist. The movie explores an allegory for substance abuse, ageism in the entertainment industry, the male gaze, and makes for a brilliantly camp feminist horror. Be aware that a lot of angles will feel sexual in a male-gazey way (and the way Sue treats herself, like an object, will make you uncomfortable. It's supposed to). Incredible film.
