A majority of the film's cast are girls and women: Kate Coleman and her daughter Max; Esther, the orphan; Kate's therapist, Dr. Browning; Kate's mother-in-law, Grandma Barbara; and two nuns at the orphanage, Sisters Abigail and Judith. In contrast, there are only three named male characters: John and Daniel Coleman, Kate's husband and son, and Dr. Värava.
The women talk to one another about a number of topics, primarily the orphan, Esther. Kate is shown arguing with her mother-in-law, confiding in (and arguing with) her therapist, and discussing Esther with her biological daughter, her therapist and the Sisters. Kate shares numerous mother/daughter moments with Max, for example, reading her bedtime stories. Kate also interacts with Esther, giving her piano lessons, talking to her about sex, and ultimately exorcising Esther from her family.
